What is a registered Provider?
As a registered Provider, Solutions 4 Support will work creatively and resourcefully with Participants to utilise their NDIS Plans to achieve their goals.
We provide professional support to assist you to navigate services and meet your needs through support, encouragement and independence.
We focus on empowering Participants to have choice and control in their life, not just their services. This may include resolving points of crisis, encouraging independence and social connection.
What is Coordination of Support?
Coordination of Support
Registered NDIS Providers are engaged to provide many support services to NDIS Participants . One such Provider category is to help the Participant coordinate the implementation and management of a NDIS plan.
This means supporting the Participant to:
- arrange any assessments required to determine the nature and type of funding required (e.g. determine the type of individualised support required)
- access a number of mainstream, community, informal and provider options
- assist in choosing preferred options and / or providers
- develop service and support agreements
- liaise with any key people in the development of plans and support
- enhance capacity to coordinate support
- resolve problems or issues with supports
- understand responsibilities under service agreements
Solutions 4 Support (S4S)
As a NDIS Participant, you can engage Solutions 4 Support to assist you coordinate the implementation and management of your NDIS Plan.
S4S will openly and transparently engage with you to assess if S4S has capacity to provide service and supports. If the Participant engages S4S, a contract of service will be entered into (Service Agreement).
Costs for service are independently set out in the NDIS Price Guide.
All prices are GST inclusive and includes the costs of Solutions 4 Support providing support.
